Athletics Transportation Permission Form

To accommodate unique student scheduling needs, PCD may occasionally allow student drivers to transport themselves and teammates to off-campus venues for practices and competitions. Parent/guardian permission is required before students are allowed to drive to athletic venues. By checking the appropriate boxes below and submitting this form, you confirm that you are the legal parent/guardian of the student named below and that you grant the permissions as indicated.
